CT shows hypodensities in the thalami bilaterally, and in the left insular cortex - the latter suggestive of an early infarct.
MRI done the next day shows hemorrhagic transformation in the insular lesion as well as bilateral thalamic infarcts with petechial hemorrhages. The distribution of the thalamic infarcts is typical of Artery of Percheron infarct.
